Self Referral
There is no requirement for a medical referral for a mammogram if you are paying the account yourself however we will require details of your Doctor to send the results to. All other X-Ray and ultrasound referrals require a medical referral eg Doctor, Physio etc.
A letter of guarantee for payment is required if you are expecting a third party eg employer, football club etc to pay for your treatment.
